“Ventral Onlay” technique is based on engraftment of buccal mucosa directly with the edges of urethral mucosa cut ventrally (anteriorily) through the spongiosum and covering the graft with the spongiosum over the sutured graft. This kind of surgery is possible to perform in patients with stricture at the proximal bulbar level without any earlier urethral injury, best in young patients with idiopatic stricture. The success is based on the thick spongiosum with excellent urethral blood suply at this level, so the graft can be taken very easy. Essential for good outcome is proper patient selection with idipatic stricture without history of excessive injury and if possible without any DVIU before.
